What’s it about?

We’re looking for people living with dementia in Scotland to fill in this short 10 question survey to help inform our campaign for Dementia Awareness 2017. We’d like to find out how dementia impacts your social life, your friendship circles and the kinds of activities you enjoy.

Who can fill it in?

The survey can be filled in by people who have dementia, their partner or other close friends and family members.

We would like to gather responses directly from people with dementia but we know that some people with dementia may need to be supported to do this. We also recognise that partners, families, carers and close friends will also want to share their valuable experiences if the person with dementia that they support is no longer able.

We want to gather responses from people who are currently living with dementia or have lived with dementia recently (within the past 2 years).

The survey is completed anonymously.  However, if you would like to share your story as part of the campaign then you can leave your contact information at the end of the survey and one of our team will be in touch to discuss further.
