This example includes an optional ‘springing’ clause so that it can be used only after two doctors certify the person as mentally incapable of running his or her own affairs.
Blank combined power of attorney (48kb)
If you want to appoint the same person or people to be your attorney for both financial and welfare decisions, you can use a combined power of attorney. You must attach two separate certificates, one for the welfare and one for the financial (continuing) powers.
