Dumfries and Galloway – Memory Clinic Link Worker

Salary Scale: Salary Scale £23,080 -£25,550 pro-rata
14 hours per week– permanent post.
Based in Dumfries you will provide information and support to people with early stage dementia and their carers at the point of and following diagnosis. You will hold a professional qualification in social work, health care or social care and have experience of working with people with dementia and their carers in a community based setting. You will be a core member of the Community Mental Health Team for Older People in Nithsdale and therefore the ability to work as part of a team and to initiate and sustain effective relationships within an interagency environment is essential.

Access to a car would be essential for this post.
